History:

 

The organization was created in 1984. It was developed to meet the hospice and home care needs of Fairfield County and surrounding counties.  Services are provided through seven programs: Hospice of Fairfield County, Hospice of Hocking County, Hospice of Perry County, Caregivers of Fairfield County, Senior Care Service, The Center for Hope, and The Hope Chest.

 

The Hospice program was started in 1984 with the formation of Hospice of Fairfield County to address palliative and support care needs to individuals with a terminal illness.  It was the fourth hospice program certified in Ohio.  The focus of hospice is to help patients carry on an alert, pain-free life and to enhance their quality of life.  In 2001, Hospice of Hocking County merged with the organization.  In addition, Hospice of Perry County was formed.  The program remains the only Hospice located in Fairfield County. No one is denied care for an inability to pay.

 

The Center for Hope was formed in 2001 to consolidate the grief support services into a single location.  The services consist of specialized grief support programs for adults, children and adolescents.  The services are available to all community members of Fairfield, Hocking, Perry County and surrounding areas.

 

The Caregivers program was started in 1988 to meet personal care and homemaking needs.  Services are provided by nurses, home health aides, companions and homemakers.  Services are customized to meet the needs of the client.  Caregivers is a provider of services through government programs such as Passport, Veterans Administration, Waiver IV, and the Ohio Department of Health.  The program's goals have always been to allow people to stay in their home.

 

The Senior Care Service program was started in 1978, prior to the development of Hospice & Health Services, Inc. as an organization.  Similar to Caregivers, the program's focus is personal care and homemaking services.  Its target population is individuals over 60 years of age.  It is funded primarily by Title IIIB and Title XX.  It is the only program of its kind in Fairfield County.  The number of people served annually has fluctuated based on funding availability.

Memorial Fund:

The primary focus of the Hospice & Health Services Memorial Fund is to support under compensated hospice care.  As part of our mission, we do not turn anyone away related to their inability to pay for services.  In addition, the fund supports Grief Services and Volunteer activities and special project requests.  The Memorial Fund has provided annually over $50,000 in financial support.  Without that support, Hospice & Health Services, Inc. would have had a difficult time fulfilling its mission.

Did you know that Hospice & Health Services, Inc.:

  1. Has an Ethics Committee to handle concerns or problems perceived as being ethical in nature.

  2. Monitors all patients and employee infections to prevent the spread of disease.

  3. Has a monitoring process to evaluate and improve all aspects of their performance.

  4. Has a committee made of community representatives to help them evaluate their services and the needs of the community.

  5. Is accredited by the Joint Commission on Accreditation of HealthCare Organizations (JCAHO).

  6. Board of Trustees is made up of volunteers from the community.
