Program
Overview
Caregivers
of Fairfield County provides skilled nursing care, personal care,
companion, and homemaking services covering all of Fairfield County.
Care is coordinated by a Registered Nurse and the patient's physician is
provided with a Plan of Treatment every 60 days. All residents of
Fairfield County are eligible for service. Services can range from help
with a bath to meal preparation to light housework to medication
reminder calls. Referrals for services can be made by anyone.
The
biggest challenge encountered by the Caregivers program in 2001 were the
recruitment and retention of sufficient, qualified Home Health Aides to
meet the community's needs and a decrease in funding.

Accomplishments

At
the Annual Awards Banquet in November, 1 Home Health Aide was
recognized for 5 years of service.

Satisfaction
surveys have been returning with excellent reviews which is evidence
of the fine care the Caregivers provide to their clients.

The
Annual COAAA survey was conducted in May with no deficiencies noted.

Development
and implementation of preceptor program for Home Health Aides.
4 CFC Home Health Aides were selected and class was taken in August
2001.

New
contract with Meals on Wheels Caregiver Support Program.

Aides and 1 Companion on staff.

Holds monthly care conferences
to evaluate the services being provided.

Home Health Aides to attend 12 education inservices annually.
