Senior Care Service

 

Program Overview

 

Senior Care Service provides personal care and homemaking services for individuals over 60 years of age.  The individual must be a resident of Fairfield County, have one or more functional limitations and need assistance to remain in their own home.  In addition, referrals are prioritized.  Preference is given to individuals who are 75+ years of age, live alone, are handicapped or in a poor state of health, live in a rural area, are a minority and/or low income.  Referrals can be made by anyone including the individual themselves.  Care is coordinated by a Registered Nurse and provided by Home Health Aides.  A Physician Information sheet is sent to the client's physician every six (6) months to keep them informed of services being provided.

 

Funding for the program is through Title IIIB of the Older American Act, contracted through the Central Ohio Area Agency on Aging (COAAA), Title XX of the Social Security Administration, contracted through the Fairfield County Department of Human Services, Alzheimer Respite Program and local match funds from the Fairfield County Commissioners.

  1. Continued development of homemaking only services.

  2. Active involvement in the Older Adult Network.

  3. Successfully passed the Central Ohio Area Agency on Aging audit with no deficiencies in May.

  4. Grant renewed to provide Alzheimer respite.

What is Senior Care Service?

 

Senior Care Service is available to persons in need of personal care and light housekeeping assistance to remain in their own homes.  Services include:

  • Bathing/Showering

  • Shampooing Hair 

  • Shaving

  • Dressing

  • Assisting with Transfers/Ambulation

  • Oversee Meal Planning

  • Laundry

  • Light Housekeeping Chores

   

Who is eligible for this service?

 

All residents of Fairfield County who are over 60 years of age, ill, handicapped, disabled or confined to their home.  Eligibility is determined by:

 

  • Proof of Age

  • Income verification

   

How often and what kind of service is available?

 

A registered nurse will visit you to ask questions and assess your needs.

 

A certified home health aide will be assigned to provide services based on your needs.  Frequency of these visits are determined by the nurse and coordinated with you, your physician and your home health aide.

 

The registered nurse will visit every other month to determine if service is appropriate, assess your health care status, and provide a progress report to your physician as needed.

  • Social Service Agencies

  • Home Health Agencies

  • Family

  • Individuals

 

How is this program funded?

 

Title IIIB of the Older Americans Act and State Block Grant monies through the Central Ohio Area Agency on Aging.

 

Title XX, the State of Ohio’s Social Services Program, through federal and state reimbursement with a contract through The Fairfield County Department of Human Services.

 

Local matching funds required to utilize Title IIIB monies have been provided by the Fairfield County Commissioners.

 

Contributions and donations are gratefully accepted and are tax deductible.

Donations from recipients are encouraged according to ability to pay. (See chart for guidelines).

 

No one will be denied service because of inability to make a donation.

Who is responsible for this program?

 

Senior Care Service is a program under Hospice & Health Services, Inc., a not for profit corporation with a Board of Trustees comprised of local individuals interested in the health needs of the community.
